1. Mark clearly "BBFE".
2. State whether this is the source or exposed person.
3. NHI of the other person involved in this incident (source or exposed)
4. Name of designated person wanting the results.
5. Phone number of designated person to inform the results have been released.
5ml Gold top tube from both the exposed and the source person.
