Unless all five requirements are on the form and both specimens (source and exposed) are received in the laboratory, the specimen will NOT be processed until the next working day.
1. Mark clearly "BBFE".
2. State whether this is the source or exposed person.
3. NHI of the other person involved in this incident (source or exposed)
4. Name of designated person wanting the results.
5. Phone number of designated person to inform the results have been released.
