Think carefully before repeating rheumatology and immunology tests. Do not repeat immunology tests in known positive patients (for example ANA, RF) on every admission.

If the test is strongly positive there is little benefit in repeating it ever. If a weak positive it could be repeated at 6 months. If negative we might repeat again at 6 months if we felt RA was likely.

Once decided if positive or negative for a patient there is no reason to repeat the test unless there is some major clinical change.

  • Do not request anti-CCP unless there is a clinical suspicion of an inflammatory polyarthritis that might be rheumatoid arthritis. In an inflammatory mono-arthropathy only request anti-CCP if other causes such as infection, gout and reactive arthritis are considered to be unlikely.
  • Do not repeat anti-CCP or Rheumatoid Factor without careful consideration.
