Diagnostic Use Or Instructions

ACE is not useful as a diagnostic test for sarcoidosis, being elevated in only 60 – 70% of patients with sarcoidosis.
Insertion (I) and deletion (D) polymorphisms explains about 50% of the variation in ACE levels.
Patients with DD have highest levels, ID intermediate and II lowest ACE levels.
The value of monitoring the ACE level to assess the course of the disease remains unclear.
ACE can also be elevated in the following conditions:
Asbestosis, Beryllium disease, Coccidioidomycosis, Diabetes mellitus, Gaucher disease, Hodgkin disease,
Hypersensitivity pneumonitis, Hyperthyroidism, Leprosy, Lung cancer, Primary biliary cirrhosis, Sarcoidosis, Silicosis, Tuberculosis.
